Northern Territory

A copy of DNA testing from all parties and a removal of father form plus an add father from for the biological father and all ID’s from all parties, plus a statutory declaration stating why the incorrect father was listed on the birth registration statement is required.

There is correction fee of $49.00 and a new certificate fee $49.00 required.

If any certificates have been issued they will need to be returned to the Births, Deaths and Marriages office before a new certificate can be released.
